Include ONLY the information that you want entered into the Internet Alumni database. All information will be entered into the WEB site and available to all persons accessing Mater Dolorosa's Internet WEB site (WWW.MDSSF.ORG). This information will not be sold by Mater Dolorosa's School.

Instructions:
  • Fill out all appropriate fields.
  • All fields in 'Red' are required fields and must be completed.
  • After all requests have been entered, select the 'SUBMIT Alumni Request' button at the bottom of this form (the fields entered will be E-Mailed to the Mater Dolorosa's Alumni Coordinator).
  • You will receive a telephone call from the Alumni Coordinator to verify your intention to add your information to the WEB site.
