Dear
Parent,
Thank you for your interest in our
Children's Faith Formation Program at
Mater Dolorosa Parish. Grades One through
Six meet each Tuesday afternoon from 3:45
to 5:00pm or 6:00 to 7:15 pm in the school
building. We have a Christmas and Easter
Recess. Grades Seven and Eight meet on
scheduled Tuesday evenings from 7:30 until
8:30pm in the school.
Students preparing
for the Sacraments of Eucharist,
Reconciliation, and Confirmation must be
enrolled one year prior to the year
of preparation for the
sacrament.
We MUST receive a
copy of the child's BAPTISMAL CERTIFICATE
and the registration form BEFORE your
child is accepted into our
program.

forms are available in the Church
lobby, rectory, Children's Faith
Formation office or select button
below to display and print the
form. You can also request the
office to mail the form to you.
Forms should be returned to the
office by September
2nd.

Parents are
expected to be registered in
Mater Dolorosa Parish and
practicing Catholics. To become a
parishioner, please select this

then print it.
