Dear
Family,
Thank you for your
interest in our Child Faith Formation
Program at Mater Dolorosa Church. Grades
One through Six meet each Tuesday
afternoon from 3:45 to 5:00pm or 6:00 p.m.
to 7:15 pm in the school building. Grades
Seven, Eight and Nine meet on scheduled
Tuesday evenings from 6:00 until 7:15 p.m.
in the school. We have a Christmas and
Easter Recess.
Students preparing
for the Sacraments of Eucharist,
Reconciliation, and Confirmation must be
enrolled one year prior to the year of
preparation for the
sacrament.
We MUST receive a
copy of the child's BAPTISMAL CERTIFICATE
and the registration form BEFORE your
child is accepted into our
program.
Registration forms
are available in the Church lobby,
rectory, Child Faith Formation office or
select button below to display and print
the form. You can also request the office
to mail the form to you. Forms should be
returned to the office by August 15,
2011.
